OCRI Awards – Submission Requirements
All nominations should include:
- Completed nomination form
- Statement of nomination (two pages maximum) explaining
why the nominee should receive the award
- Short profile (four pages maximum) including:
- a brief description of the nominee's operations including
materials outlining those activities that address the award
being considered;
- the nominee's impact on promoting achievement in that
area, and activities highlighting overall significant achievements;
and
- a current corporate fact sheet or CV of the nominee.
Please note the following:
- Nominations must be completed and submitted by 4:00 p.m.
on December 5, 2008
- Judges may contact nominees for more information or explanation
- Nominees must be available for video interview
Format requirements:
- All submissions must be on 8.5" x 11" paper,
one sided, typed in 12 pt, Times New Roman font.
- Nomination package must not exceed six pages - please
do not provide any collateral beyond the aforementioned
profile, impact statements and current CV.
- To facilitate reproduction, newspaper clippings should
be attached to 8.5" x 11" paper or photocopied
on 8.5" x 11" paper
- Emailed documents should be accompanied by a faxed or
mailed copy of the nomination form signed by all required
parties.

Completed nomination packages must be submitted by 4:00 p.m.
on December 5, 2008 with the exception of nominations for
the High School IT Entrepreneur of the Year Award (please
contact the appropriate school board for deadline details).
